This position provides community-based direct care, service coordination, and support to individuals located within the community.
Responsibilities and Duties
Duties include but are not limited to:
- Ensure ongoing program compliance according to the regulatory standards of the housing program and grant requirements administered by the program.
- Maintain accurate and complete client and program records to ensure all grant requirements are met, documentation is properly completed, and reports are submitted in a timely manner.
- Foster and maintain positive, professional relationships with area property management owners, companies, and housing authorities.
- Conduct client intakes, assessments, and complete all required documentation in compliance with program standards.
- Develop and implement individualized serve plans in collaboration with program participants, aligned with their goals, strengths, and desired outcomes.
- Participate and actively collaborate with the interdisciplinary team to ensure program participants receive the support and resources they need for long-term success.

Job Requirements:
- Requires a Bachelor's degree or equivalent life and/or professional experience.

- Excellent assessment, counseling, and training skills.
- Knowledge of housing insecurity, homelessness, and related housing assistance programs and grants.

A combination of approved hours of experience and education: One year (1,560 clock hours) of child development experience in a nursery school, kindergarten, or licensed day care center and 30 semester hours of credits from an accredited college or university with six semester hours in courses related directly to childcare and/or child development, from birth to age six.
